You, on behalf of you and your child, have registered
on the MDA Carnival website and have agreed to
MDA’s
Terms and Conditions, which includes
an indemnification statement and waiver, release
and consent provision.
MDA
requires that the Host Family accept and sign
the MDA
Carnival Host Family Waiver, Release and Consent
Form prior to hosting a Carnival
to benefit MDA.
MDA
also requires all Carnival participants and
parents/guardians of Carnival participants who
are under the age of 18 sign the
MDA Carnival Participants Waivers, Release and
Consent Forms.
Be
sure to review and abide by the Safety
section of the Web site for details on how to
have a safe and successful Carnival.
Please
submit the original MDA Carnival Host Family
Waiver, Release and Consent Form and the MDA
Carnival Participants Waiver, Release and Consent
Form with all carnival proceeds in the form
of check or money order to:

Be
sure to also complete an MDA
Carnival Host Family Summary Form
which includes your name, mailing address, how
many people came to your carnival and the total
amount of money your family raised for Jerry’s
kids®, along with any great ideas you’d
like to share for the MDA Carnival Program.
MDA
will credit Carnival proceeds you collect to
the local MDA office in your community. Your
Carnival will directly help families in your
area who are affected by neuromuscular disease!
